Food, movement and sleep for PCOS (now called PMOS), without the shame
Healthy eating and movement help everyone with PMOS, formerly PCOS, at any weight. Here is what the evidence actually says, in Filipino terms, with no calorie counting and no forbidden foods.

Quick answer
The 2023 International Guideline recommends healthy eating and physical activity for everyone with PCOS, whatever their weight (Rec 3.1.1), and says no specific diet or exercise type is better than another (Rec 3.3.1, 3.4.1). Pick what you can keep doing.
Lifestyle support helps insulin resistance, energy, mood and cycles. It is a foundation that works alongside medicine, not a test you have to pass first.

“Ang hirap pumayat kahit anong gawin ko.” It is not a willpower problem
Many Filipinas with PCOS say they have tried dieting and light workouts and their weight stays the same or goes up. That is a real effect of insulin resistance, which is common in PMOS at every body size. It is not laziness and it is not your fault.
That is also why we do not set weight targets here. The guideline asks doctors to screen for eating disorders regardless of weight (Rec 2.5.1), because restrictive dieting can do harm. Health markers like your OGTT, lipids, blood pressure, energy, mood and cycles are better ways to see progress.
“Ang hirap pumayat kahit anong gawin ko.”
I can't lose weight no matter what I do.
Insulin resistance makes this genuinely harder; it is not a willpower problem. Healthy eating and movement help everyone with PCOS at any weight, and no single diet is best (Rec 3.1.1, 3.3.1). Doctors are also asked to screen for eating disorders regardless of weight (2.5.1).
“Parang ako pa may kasalanan na may PCOS ako.”
Doctors shame me about my weight.
It is not your fault. The 2023 Guideline says weight stigma is common and harmful, and asks clinicians to recognise their own weight bias (Rec 3.6.1, 3.6.2). You are allowed to ask for respectful, guideline-based care, or to see someone else.
Food: add, don't forbid
No diet is proven best for PCOS. What tends to help insulin is balance, not restriction. A simple way to think about a plate:
- About half vegetables: pinakbet, ginisang kangkong, ampalaya, the gulay in sinigang and tinola.
- About a quarter protein: inihaw na isda, tokwa, itlog, monggo, chicken.
- About a quarter carbs: rice is fine. Brown rice, adlai, kamote, saba and corn add fibre if you like them.
Pairing carbs with protein and vegetables slows how fast blood sugar rises. You do not need to give up rice, mango or merienda. Try the Filipino PCOS Plate Builder or print the plate guide.
A note on supplements
Inositol is a food supplement with limited evidence, and no product replaces meals. See our honest inositol guide.Movement that fits Filipino life
Any movement counts, and no type is better than another. Choose what fits your day and your budget:
- Walking to the jeep or tricycle stop, or a mall walk when it is too hot outside.
- Zumba or dance videos at home, badminton with barkada, swimming.
- Strength moves at home using water bottles or a backpack with books.
Benefits show up in insulin sensitivity, mood and sleep, even when the scale does not move.
Sleep and mood
Depression and anxiety are more common with PCOS, and the guideline asks doctors to screen all adults (Rec 2.2.1, 2.2.2). Poor sleep also makes cravings and energy dips worse. If snoring, gasping at night or heavy daytime sleepiness sound familiar, mention them to your doctor. Read more on mood and anxiety with PCOS.
When doctors talk about weight
Filipinas on r/PCOSPhilippines report doctors who mocked their weight or said the only answer was “pills and lose weight.” The 2023 Guideline says weight stigma is common and harmful and asks clinicians to recognise their own bias (Rec 3.6.1, 3.6.2). You can ask for specific tests, ask what else can help, or see another doctor. Our Consult Prep Pack has a respectful script.

Frequently asked questions
Do I have to lose weight to treat PCOS?
No. Healthy habits help at any weight, and medicines for cycles, skin, hair and fertility work regardless of weight. Your doctor should not make treatment depend on weight loss.
Is keto or intermittent fasting best for PCOS?
The guideline found no diet better than another. Very restrictive diets can be hard to keep and can harm your relationship with food. Choose a pattern you can live with.
Is PMOS the same as PCOS?
Yes. PMOS (polyendocrine metabolic ovarian syndrome) is the new name for PCOS, announced in The Lancet on 12 May 2026. It is the same condition with the same diagnosis and the same treatments. Both names will be used until the 2028 International Guideline update.
