Quick answer

Four free A4 sheets for PCOS (now called PMOS): a check-up checklist with current Philippine prices, a 12-month cycle diary, questions for your OB-GYN in English and Taglish, and a one-page Filipino plate guide. Each page has a “Print or save as PDF” button.

How to print or save as PDF

Open a printable and tap Print or save as PDF. On a phone, choose “Save as PDF” in the print screen to keep it in your files. On a computer, pick your printer or “Save as PDF”. Every sheet is sized for A4 paper, the standard size at Philippine print shops and offices.

Prices on the sheets come from our monthly price data, so print a fresh copy before each consult.
