Birth control pills for PCOS (now called PMOS): Diane vs Althea vs Yaz
“Diane or Althea?” is the most-asked treatment question in Filipino PCOS groups. Here is the honest answer: they are the same formula. This guide covers every pill Filipinas ask about, what PMOS, formerly PCOS, care guidelines say about them, and what they cost.
Editorially reviewed on October 29, 2026. [TODO: medical reviewer] How we review
Quick answer
Combined pills are a common first treatment for irregular periods, acne and unwanted hair in PCOS. Diane-35, Althea and Cybelle contain the same hormones (cyproterone acetate 2 mg plus ethinylestradiol 35 mcg), so Althea is usually the cheaper twin of Diane-35.
The 2023 International Guideline says no pill type is preferred overall, and that this 35 mcg cyproterone formula should be second-line, mainly because of clotting risk (Rec 4.2.5). If you are on it, do not stop on your own. Ask your doctor whether another pill suits you. All combined pills are prescription-only in the Philippines.

Diane or Althea? They are the same formula
Filipinas on r/PCOSPhilippines ask this every week: “Switching from Diane 35 to Althea for PCOS, what should I expect?” and “Mas mura kasi Althea pills kaya ko sya cinoconsider uli.” (thread, thread.)
The answer: Diane-35 and Althea both contain cyproterone acetate 2 mg plus ethinylestradiol 35 mcgCPA/EE: Cyproterone acetate plus ethinylestradiol, the formula in Diane-35 and Althea., per their Watsons and Southstar listings. Cybelle is sold with the same formula. The hormones and amounts match, so the difference you pay is brand, not strength. Diane-35 costs about ₱695.75 per box at Southstar; Althea costs ₱487.45 per box to ₱540 per box.
Yaz, Yasmin and Lizelle are a different family. They use drospirenone 3 mg as the progestin. Yaz has 20 mcg of ethinylestradiol; Yasmin and Lizelle have 30 mcg. Filipino users describe Lizelle as the local counterpart of Yasmin.
Switching between twins
Same formula does not always mean an identical experience; fillers and how your body adjusts can differ. Switching brands, or switching to a different formula, is a conversation to have with your doctor, who can tell you how to change over without a gap.

Diane-35, Althea and Cybelle have the same hormones in the same amounts. Brand price differences are not differences in strength. The 2023 Guideline calls this 35 mcg CPA formula second-line, mainly because of clotting risk. If you are on it, do not stop suddenly: ask your doctor whether another pill suits you.

What the 2023 Guideline says
- Combined pills could be recommended for irregular cycles and for hirsutism (unwanted hair), and no specific type can be recommended over another (Rec 4.2.4).
- 35 mcg ethinylestradiol plus cyproterone acetate preparations should be considered second-line (Rec 4.2.5), mainly because of the risk of blood clots. In the Philippines this formula (Diane-35, Althea, Cybelle) is often the first prescription.
- For hirsutism specifically, the combined pill is the preferred medicine (Rec 4.4.1).
What this means for you: second-line does not mean dangerous or wrong for you. It means the guideline would usually try another combined pill first. If you are already on Diane-35 or Althea, keep taking it and ask your doctor at your next visit: “Is a different pill an option for me, given my clotting risk?” Do not stop suddenly on your own; stopping mid-pack can bring on bleeding and leaves you without contraception.
How the pill helps PCOS
In PCOS, the ovaries often make more androgensandrogens: Hormones like testosterone. Everyone has them; in PCOS they are often higher. and release eggs less often. A combined pill switches off the ovary's monthly cycle and lowers androgen activity. That is why it can bring predictable bleeds, fewer breakouts and slower hair growth. The regular withdrawal bleed also protects the womb lining, which matters if your periods are months apart.
The pill manages symptoms while you take it. It does not change the underlying condition, which is why some women feel their PCOS “came back” after stopping. That is expected, not a failure. Pills do not address insulin resistance, so the guideline still asks for a 75 g OGTT and a lipid profile, whatever pill you take.
Who it suits, and who should not take it
The combined pill often suits women with PCOS who:
- want regular, predictable periods and protection of the womb lining;
- have acne or unwanted hair from high androgens;
- also want contraception (remember: PCOS is not contraception).
A doctor must screen you first. All combined pills are prescription-only in the Philippines. Doctors check for migraine with aura, smoking if you are over 35, a past blood clot or a family history of clots, and high blood pressure, among other things. Buying “Diane 35 Thai” or other pills without a prescription skips that screening.
If you are trying to get pregnant, the pill is not the right tool; see the fertility guide. For a deeper comparison of pills and their risks, our sister site birthcontrolphilippines.com covers every brand.
Side effects and fears
Early side effects are common and often settle within the first two or three packs: spotting, breast tenderness, nausea and mood changes. Filipinas on r/PCOSPhilippines describe their first month on Althea with lines like “Less oily na face ko” and “Naging antukin” (thread). Those are personal reports, not evidence, but they show you are not alone. Tell your doctor about mood changes that worry you.
“Diane or Althea? Alin ang mas okay?”
Diane, Althea or Yaz: which pill is better?
Diane-35 and Althea contain the same hormones (cyproterone acetate 2 mg plus ethinylestradiol 35 mcg). The guideline says no pill type is preferred overall, and that this specific 35 mcg formula should be second-line, mainly because of clotting risk (Rec 4.2.5). Ask your doctor whether another pill suits you; do not stop suddenly on your own.
“Pwede ba ako bumili ng pills kahit walang reseta?”
Can I just buy pills over the counter?
All combined pills are prescription-only in the Philippines. A doctor needs to screen for things like migraine with aura, smoking over 35, clotting history and high blood pressure first.
“Delikado ba ang matagal na pag-inom ng pills?”
Is long-term pill use dangerous?
It is a fair question with a balanced answer. Talk through your own risks with your doctor; birthcontrolphilippines.com explains pill risks in detail.
“Hindi ba ako mabubuntis kasi may PCOS ako?”
Is PCOS a form of contraception?
No. PCOS means you ovulate less often, not never. If you do not want to get pregnant, you still need contraception.
“Bumabalik yung tagyawat kahit naka-pills.”
My acne keeps coming back even on pills.
PCOS acne is androgen-driven. If one option is not working after a fair trial, ask about others: a different pill, topical treatments, a dermatologist, or spironolactone with effective contraception.
Cost per month
One box covers one cycle. Diane-35 works out to about ₱695.75 a month and Althea to about ₱487.45 to ₱540. PhilHealth Konsulta does not list oral contraceptives among its covered medicines, and the PH study of Filipino women with PCOS found that cost was one reason people stopped. See the full cost guide.

Safety
Signs of a blood clot: get help now
- Pain or swelling in one leg, especially the calf
- Sudden chest pain or shortness of breath
- Sudden severe headache, vision changes, or weakness on one side
Who needs extra screening
Migraine with aura, smoking over 35, a past clot or a family history of clots, high blood pressure, and some liver conditions. Your doctor will ask about these before prescribing.
Do not stop suddenly
If you are worried about your pill, keep taking it until you have spoken to your doctor. Stopping mid-pack can cause bleeding and ends your contraception.
Buying pills online
Rx medicines may only be sold online by licensed pharmacies with a physical address, against a prescription (FDA Advisory 2022-0108). Marketplace sellers without a prescription check are a red flag.

Frequently asked questions
Diane or Althea: alin ang mas okay?
They have the same hormones in the same amounts: cyproterone acetate 2 mg plus ethinylestradiol 35 mcg. Althea is usually cheaper. The 2023 Guideline calls this formula second-line because of clotting risk, so ask your doctor whether a different pill suits you.
Is Cybelle the same as Diane-35?
Cybelle is sold with the same formula as Diane-35 and Althea (cyproterone acetate 2 mg plus ethinylestradiol 35 mcg). We have not yet verified its current price.
Is Yaz better than Diane for PCOS?
The guideline does not rank one pill type above another, except that the 35 mcg cyproterone formula should be second-line. Yaz uses drospirenone with 20 mcg of estrogen. Which suits you depends on your history, so it is a decision to make with your doctor.
Should I stop my Diane or Althea because it is second-line?
No, not on your own. Second-line means the guideline would usually try another pill first. Keep taking it and ask your doctor whether switching makes sense for you.
Pwede ba bumili ng pills kahit walang reseta?
All combined pills are prescription-only in the Philippines. A doctor needs to check things like migraine with aura, smoking, clotting history and blood pressure first.
Does PhilHealth cover birth control pills for PCOS?
PhilHealth Konsulta's benefit table does not list oral contraceptives. Some HMOs may cover consults and labs; medicines vary by plan.
Will I gain weight on the pill?
Some women report weight changes or mood swings on the pill, and others notice none. If a side effect bothers you, tell your doctor; there are other options.
Is PMOS the same as PCOS?
Yes. PMOS (polyendocrine metabolic ovarian syndrome) is the new name for PCOS, announced in The Lancet on 12 May 2026. It is the same condition with the same diagnosis and the same treatments. Both names will be used until the 2028 International Guideline update.
