HOMA-IR Calculator for PCOS (now called PMOS)
Editorially reviewed on October 29, 2026. [TODO: medical reviewer] How we review
Quick answer
HOMA-IR is fasting glucose (mmol/L) times fasting insulin (uIU/mL), divided by 22.5 (Matthews et al. 1985). Higher numbers suggest more insulin resistance. There is no single cut-off that fits everyone, and for PCOS the 2023 Guideline prefers the 75 g OGTT to check blood sugar status. A 75 g OGTT costs about ₱500 per test at some Hi-Precision branches.

How it is calculated
HOMA-IR = fasting glucose (mmol/L) × fasting insulin (uIU/mL) ÷ 22.5
With glucose in mg/dL, we first divide by 18.016 to convert to mmol/L. (Matthews et al., Diabetologia, 1985.)
Questions to ask your doctor
- Should I have a 75 g OGTT instead of, or as well as, this test?
- What does my HOMA-IR mean with my lab's insulin method?
- Do my results change what treatment or lifestyle support you suggest?
- When should we recheck my blood sugar and lipids?
Educational only. Not a diagnosis. Talk to a doctor about your results.
How it works
The formula. HOMA-IR (homeostatic model assessment of insulin resistance) was described by Matthews and colleagues in 1985. It uses two numbers from one fasting blood test: glucose and insulin. With glucose in mmol/L, HOMA-IR equals glucose times insulin divided by 22.5. Philippine labs usually report glucose in mg/dL, so the calculator first divides by 18.016 to convert to mmol/L. Insulin is reported in uIU/mL, which is the same as mIU/L.
What the number means. Insulin resistance means your body needs more insulin to keep blood sugar steady. It is common in PCOS (PMOS, formerly PCOS) at any weight, and it is one reason PCOS raises the risk of prediabetes and type 2 diabetes. A higher HOMA-IR suggests more insulin resistance.
The honest caveats. There is no universal HOMA-IR cut-off. Insulin tests are not standardised between labs, and studies use different thresholds for different populations. In a Filipino tertiary-hospital sample, women with phenotype A PCOS had a median HOMA-IR of 4.70 (Bisquera et al., JAFES 2023), but that is a study average, not a target or a diagnosis. HOMA-IR is not used to diagnose PCOS.
What the guideline prefers. The 2023 International Evidence-based Guideline calls the 75 g oral glucose tolerance test (OGTT) the most accurate test of blood sugar status in PCOS (Rec 1.9.9), recommended at diagnosis and every 1 to 3 years. It also recommends a lipid profile at diagnosis regardless of age and weight (Rec 1.8.3). PhilHealth Konsulta lists OGTT, HbA1c, fasting blood sugar and a lipid profile among its covered tests, depending on your provider.
Use it to start a conversation. Bring the number and the questions under the calculator to your OB-GYN or endocrinologist. Your doctor will read it with your OGTT, lipids, blood pressure and symptoms.

Frequently asked questions
What is a normal HOMA-IR?
There is no single normal value that applies to everyone. Cut-offs vary by lab method and population. Ask your doctor how to read your number with your lab's insulin test.
Kailangan ko ba ng HOMA-IR para ma-diagnose ang PCOS?
No. HOMA-IR is not part of PCOS diagnosis. The 2023 Guideline recommends a 75 g OGTT to check blood sugar status in PCOS.
My glucose is in mg/dL. Does that work?
Yes. Choose mg/dL and the calculator converts it to mmol/L before applying the formula.
Can I put this calculator on my website?
Yes. Use the embed code on this page and keep the credit link.
Is PMOS the same as PCOS?
Yes. PMOS (polyendocrine metabolic ovarian syndrome) is the new name for PCOS, announced in The Lancet on 12 May 2026. It is the same condition with the same diagnosis and the same treatments. Both names will be used until the 2028 International Guideline update.
